AfHNS guidelines for evaluation of a thyroid nodule / mass

slide24-1.png

  • After addressing possible thyrotoxicosis, U/S may assist to differentiate a single hyper-vascular (potentially hyperfunctional) nodule vs. diffusely hypervascular goiter (Graves disease)
  • Other than with a rapidly growing thyroid mass, biopsy of a thyroid nodule is typically not urgent, and referral to a better resourced center for FNAC is recommended
  • If not possible, reliance is placed on clinical and, if available, U/S findings to risk-stratify using the Thyroid Imaging Reporting and Data System (TI-RADS)
  • Patients with high risk clinical features are advised to have surgery with histological review of the thyroid specimen
