AfHNS guidelines for evaluation of a thyroid nodule / mass

  • Wayne’s Index
  • After addressing thyrotoxicosis, U/S may assist to differentiate a single hypervas-cular (potentially hyperfunctional) nodule vs. diffusely hypervascular goiter (Graves disease)
  • Other than with a rapidly growing thyroid mass, biopsy of a thyroid nodule is typically not urgent, and referral to a better resourced center for U/S and FNAC is recommended
  • If ultrasound and FNAC are not possible, reliance is placed on clinical features to risk-stratify
  • Patients with high risk clinical features are advised to have surgery with histolo-gical review of the thyroid specimen
  • U/S can be used to risk-stratify using the Thyroid Imaging Reporting and Data System (TI-RADS)
